About The Health Information Toolkit

Low literacy and limited English proficiency are barriers to health and healthcare for many adults.

This Health Information Toolkit is designed to provide plain language health education materials created using health literacy guidelines. Created for 12 different language groups, the materials are culturally and linguistically responsive as well. There is information on general health, women’s health, patient/provider communication, and medications. You can access the health education materials by language. 

Each language page has a playlist of videos covering the following topics:

  1. General Health education videos are available in 14 languages including subtitled versions for those who are hard of hearing.

2. Women’s Health education videos are available in 12 languages.

3. Patient/Provider Communication videos are available in 12 languages.

Additionally, there are Medication Instructions for commonly prescribed medications used to treat diabetes, high cholesterol, and high blood pressure are available on the corresponding language page. These are downloadable as pdfs.
